Guatemala Registration Form
It is time again to plan for our annual Prestoncrest Guatemalan mission trip, which will be
July 4-11, 2009. Your immediate attention and action are requested. You will not be an official participant of the group until your deposit and registration form have been received in the church office.
A $250.00 deposit to secure your place on this trip will be required by April 12th and no refunds will be issued after June 8th since the church would be responsible for the remaining balance for the hotel at this point.
Dates to Remember
March 8 - 1st Meeting
April 12 - $250.00 deposit due
June 7 - 2nd Meeting/Final Balance due
After June 8 - No refunds of deposit money will be issued
This year’s targeted work groups include medical, pharmacy, children and construction. We have a specific number of people needed in each of these areas. Priority will be given to those with medical training and Spanish proficiency to fill the quantities for each of the work groups. After all spots are filled, a waiting list will be established.
Due to these needs, please assess your interest in attending and note the projected costs.
Costs
Hotel - 435.00 (standard room, double occupancy)
Insurance - 15.00
Bus drivers - 50.00
Group meal - 15.00
Total - $515.00
In addition, you will need to allow for meals, shopping, and $3.00 for the airport exit fee. You can realistically count on spending an additional $200 – $250 for food during the week.
Airfare Information
You are responsible for making your own airline arrangements to and from Guatemala. If you are planning on using airline miles to purchase your tickets do so as soon as possible since these tickets are limited. The group will be flying on American Airlines from DFW to GUA (Guatemala City, La Aurora, Guatemala) on July 4, DFW flight #2195 departing at 5:45 PM. Returning from GUA on July 11, flight #2196 departing at 8:35 AM. Transportation to the hotel is only provided for those traveling with the group on this flight.

The registration form must be received at the same time as the desposit to hold your position on the trip.
